My1Stop.com understands the need for client confidentiality. When a patient enters your office, they go to the reception desk and sign their name on the patient sign in form. The patient will then peel off the small corresponding number next to their name. My1Stop.com's Patient Sign-In sheets feature a number that has no sticky adhesive (once folded under) for the patient to handle.
Next, the receptionist will remove the adhesive strip and apply it to either the patient's chart or the call sheet. The form then exposes the words "Do Not Sign Here" for the next patient.
In many cases, this step is an improvement to the process previously followed. Instead of hand writing the information to the patient's chart, the receptionist simply applies the patient sign-in sheets adhesive strip to a folder or other record keeping form. This helps save time and reduces errors.
The second part of the patient privacy sign-in sheet is used as a daily log sheet. This assists the office in keeping track of their appointments. The second sheet can also help track the wait times of patients.
The label liner with block out allows the log sheet to stay protected by not allowing patients to see through the liner. In addition, side stubs help to prevent anyone from peeking!
